The Grant County Sheriff's Department said 46-year-old Deputy Ric Hottenstein was southbound on Highway 80 at 3:03 p.m. when 29-year-old Betty Martin pulled out from the stop sign at Clifton Road.
The sheriff's department said Hottenstein veered the 2013 left in an attempt to avoid a collision but was not successful.
Deputy K-9 Diego was in the squad kennel and was thrown forward causing a small cut and a sprained leg, the report said. Hottenstein suffered a wrist injury, bumps and bruises.
Three-year-old Melissa Martin, a passenger in Betty Martin's van suffered a large bump on the head, the sheriff's department said.
Platteville EMS arrived on scene and checked out all those involved, but did not transport anyone.
Hottenstein was taken by the sheriff to Southwest Health Center to be further evaluated and Melissa Martin was taken by family to be checked out.
K-9 Diego was taken to Family Pet Hospital in Platteville for further evaluation.
The sheriff's vehicle sustained heavy front-end damage and multiple air bags deployed, the report said. The Dodge Caravan Betty Martin was driving sustained front-end damage.
Officials said Betty Martin was cited on suspicion of failing to yield from a stop sign.
Hottenstein is from Platteville. Betty and Melissa Martin are from Livingston, Wis.
